Step3a: 

Work toward Nutritional Ketosis. Achieve a well-oxygenated, Fed State first. It is essential to assure that your body is in a fed state. Conditions that alter fed states are major surgery and infection and most likely at some time contracting COVID, having a high BMI > 28 and a chronic infectious/inflammatory state.

To be in a fed state, a pulse oxygen level of at least 95% when at rest or sitting is necessary. A fed state can be measured by getting a serum leptin. An indication that leptin is low or not useful is insulin resistance and progesterone resistance. Insulin resistance results from longstanding elevated A1C. Progesterone resistance is indicated by low serum testosterone or high dihydrotestosterone levels. 

Prolonged fasting or acute injury, infection, or surgery will lower Leptins. An optimal time to measure serum leptin is in a more steady state of metabolism. Additionally, it is important to recognize conditions that increase leptin as much as 40%; such as after an episode of overeating or decrease by 60 to 70% or after fasting or during chronic fasting. Typically, obesity raises leptin levels. However, with obesity, the body and brain become insensitive to leptin. 

The goal of a serum Leptin level in leptin-sensitive patients is 0.5 to 12.5 ng/mL for men.

Step 3b:

For a sedentary job, it is necessary to exercise frequently, i.e. every 1-2 hours, shifting from sitting to standing, walking in place, and heel/toe lifts in 2 sets of 12 reps.

Reversing Insulin resistance takes time and can be accelerated by adding strength training. A1C of <5 improves as insulin sensitivity increases.

Step3d:
